Final Site Plan Review Application Process
1. Applicant Submits a Final Site Plan Review Application to the Development Services
Department
The application must conform to the Final Site Plan Review Submittal Requirements
Checklist on page 4 of this application packet.
Planning Division staff screen the submittal at the front counter to determine
whether all required items listed in the Final Site Plan Review Submittal
Requirements Checklist have been submitted.
Applications missing required items will not be accepted for submittal
2. City Staff Conduct Detailed Completeness Check
• Planning Division staff conducts a detailed completeness check within 30 days of
submittal.
• The assigned Planner notifies the applicant in writing regarding the completeness of
the application.
• An application is not be deemed technically complete until all information necessary
to evaluate the proposed development, its impacts, and its compliance with the
provisions of the Springfield Development Code and other applicable codes and
statutes have been provided.
Incomplete applications, as well as insufficient or unclear data, will delay the
application review process and may result in denial.
3. City Staff Review the Application & Issues Final Approval
This is a Type I decision and thus is made without public notice and without a public
hearing since there are clear and objective approval criteria and/or development
standards that do not require the use of discretion.
Decisions address all the applicable approval criteria and/or development standards.
Applications may be approved, approved with conditions, or denied.
The City mails the applicant and any party of standing a copy of the decision, which
is effective on the day it is mailed.
The decision issued is the final decision of the City and may not be appealed.

Response to Conditions of Approval
Dear Melissa,
In response to Tentative Site Plan Review Major Modification (Case # 811-21-000098-TYP2) Staff Report
and Decision the following responses to the Conditions of Approval are hereby submitted for your
consideration and approval. With this submittal we believe we meet all conditions of the decision.
Major Site Plan Modification Conditions of Approval
There are 16 Conditions of Approval in the Major Site Plan Modification decision. The Conditions are in
Italics. The applicant's responses are in plain text.
Condition of Approval #1: The applicant must retain a private professional civil engineer to design the
site improvements and all public improvements in conformance with City codes, this decision, and the
current Engineering Design Standards and Procedures Manual (EDSPM) pursuant to SDC 5.17-125B
and SDC 4.1-110.B. The private civil engineer also is required to provide construction inspection services.
Applicant Response: The applicant has retained Branch Engineering to provide civil engineering services
regarding the design of improvements in compliance with EDSPM and SDC 5.17-125.6 and SDC 4.1-
110.6. Branch Engineering will also provide construction inspection services.
Conditions of Approval #2: The applicant must place new utility lines underground and provide
screening from utility vaults/transformers1connections along the Deadmond Road street frontage.
Screening must be in conformance with the standards in SDC 4.4-110. B. and be shown on the Final Site
Plan.
Applicant Response: All new utility lines will be placed underground, and the utility vault shown near the
front property line will be screened meeting the standards in SDC 4.4-110.B.
Condition of Approval #3: Prior to approval of the Final Site Plan, the applicant must provide an
Operations and Maintenance plan to the City for review. The plan must designate maintenance
responsibility for operating and maintaining the system and must be distributed to all property owners and
tenants of the site pursuant to SDC 4.3-110.B.
Applicant Response: An Operations and Maintenance plan has been generated. It can be found with this
submittal. The Notice will be recorded prior to Final Partition approval.
Condition of Approval #4: The proposed infiltration rain garden system must be fully
installed and fully functioning prior to Final Site Inspection approval. The system must be
inspected and approved by the City prior to building occupancy issuance.
Applicant Response: Acknowledged and agreed. The infiltration rain gardens shall be
fully vegetated prior to final occupancy. 7N

Condition of Approval 13: Prior to Final Site Plan, the applicant must provide Wellhead Protection Signs
and Notes from the SUB Drinking Water Protection division on both the Site Plan and Construction Notes
in accordance with SDC 3.3-200. Wellhead Protection Signs can be purchased from SUB.
Applicant Response: Springfield Wellhead Protection Area Signs and Construction Notes are noted on
the Site Plan.
Condition of Approval 14: Prior to Final Site Plan, the applicant must depict the three locations of
Springfield Wellhead Protection Area signs to be posted at the trash enclosure and the two vegetated
stormwater management facilities serving the development site in accordance with SDC 3.3-200.
Applicant Response: The three required Springfield Wellhead Protection Area signs are noted on the Site
Plan.
Condition of Approval 15: The property owner or their designee is responsible for ongoing and
perpetual maintenance of the private stormwater facilities on the site to ensure they function as designed
and intended, and to ensure protection of groundwater resources. Annual maintenance records must be
kept by the property owner or their designee and provided to the City for review upon reasonable request
— normally within five business days.
Applicant Response: Acknowledged and agreed. Annual maintenance records for the stormwater facilities
will be kept by the property owner or designee and provided to the City if and when requested.
Condition of Approval 16: Groundwater protection must be observed during construction on the site.
The applicant must maintain the private stormwater facilities on the site to ensure the continued
protection of surface water and groundwater resources.
Applicant Response: Acknowledged and agreed. The applicant will maintain the private stormwater
facilities to protect surface water and groundwater resources.

This stormwater management operations and maintenance plan was prepared for the Deadmond
Ferry development in Springfield, Oregon for the improvctnems south of Deadmond Ferry- Road
located at 287 Deadmond Ferry Road. City of Springfield regulations requires a stormwater
management system operations and maintenance plan. The stormwater management system includes
infiltration rain gardens and other miscellaneous conveyance infrastructure. The various facilities
operation and maintenance logs and schedule have been included in the appendices.

Appendix A- Notice of Operations & Maintenance Agreement
The Notice of O&M Agreement identifies the property as having stormwater management facilities
on the property and identifies the financial method used to cover future operations and
maintenance. The Notice of O&M Agreement must be completed and then recorded at Lane County
Deeds and Records. Signatures on the Notice shall be notarized. The Notice may be submitted in
person or mailed, along with payment of the applicable fees, to the County Recorder's Office, Lane
County Deeds and Records, 121 E. 8th Avenue, Eugene, OR 97401.,
http://www.co.Iane.or.us/AT_PropRec/default.htiu.
The property description on the Notice must be a full legal description of the property
and may not be a tax lot number. Legal descriptions may be obtained from the county assessor's
office.
This Notice shall be printed on legal -sized (8 54 x 14) paper to facilitate the recording
process. if printed on smaller paper, additional recording fees may apply.
Appendix B- Operations & Maintenance Agreement
The completed Form must identify the owner's name, address, and phone number, the site
address, financial method used to cover future operation and maintenance, and parties
responsible for inspecting and maintaining the facility. The Operations & Maintenance Agreement
does not need to be recorded.
A copy of the stormwater management site plan shall be attached to the Operations &
Maintenance Agreement. The plan, approved as part of the permit for development, must show
the location of the facility on the site, the sources of runoff entering the facility, and the ultimate
stormwater destination.
Appendix C- Stormwater Management Facility Inspection and Maintenance Log
Stormwater management facility inspection and maintenance logs must be kept on file by facility
owners. Logs should note all inspection dates, the facility components that were inspected, and
any maintenance or repairs made. The facility -specific O&M plans can serve as a checklist for what
should be included in the log (e.g. the facility elements that need to be inspected, frequency of
inspection, conditions that indicate maintenance is needed, etc.)

MAINTENANCE LOG
4.5.14 Soakage Trench O & M Plan
Soakage Trenches are infiltrating flood control devices. Soakage Trenches are linear
excavations backfilled with sand and gravel injecting stormwater runoff into the ground
recharging groundwater. There are various components within the system - inlet piping,
aggregate storage basin and perforated piping. The trench surface may be covered with
grating, stone, sand, or a grassed cover with a surface inlet and may also be installed under
hard surfaces such as driveways.
All facility components, vegetation, and source controls shall be inspected for proper
operations and structural stability. These inspections shall occur, at a minimum, quarterly
for the first two years from the date of installation, then two times per year afterwards, or
within 48 hours after each major storm.
Training and/or Written Guidance information for operating and maintaining Soakage
Trenches shall be provided to all property owners and tenants. A copy of the 0 & M Plan
shall be provided to all property owners and tenants.
Inspection Logs shall be kept by the facility owner demonstrating the following items have
been inspected and are being maintained properly:
• Access to the Soakage Trench shall be safe and efficient. Obstacles preventing
maintenance personnel and/or equipment access to the components of the facility
shall be removed.
• Algae Growth located on top of the soakage trench should be removed and
disposed of properly.
• Debris/Sediment that is found to clog the pipe shall be removed and disposed of in
accordance with applicable federal and state requirements.
• Failing Soakage Trenches shall be repaired and/or replaced if water is noticed on
top of trench within 48 hours of a major storm.
• Piping shall be cleared of sediment and debris to maintain conveyance. If piping is
clear of sediment and debris and yet conveyance is poorly maintained, fabric around
the pipe shall need replacing.
• Shut -Off Valve or Flow -Blocking Mechanism may have been required with the
construction of the drywell to temporarily prevent stormwater from flowing into it,
in the event of an accidental toxic material spill. This may also involve mats kept on-
site that can be used to cover inlet drains in parking lots. The shut-off valve shall
remain in good working order, or if mats or other flow -blocking mechanisms are
used, they shall be kept in stock on-site.
• Sediment in the aggregate obstructing storage and infiltration shall be excavated
and replaced.
• Vegetation such as trees should not be located in or around the soakage trench
because roots from trees can penetrate the unit body, and leaves from deciduous

trees and shrubs can increase the risk of clogging the intake pipe. Large shrubs or
trees that are likely to interfere with operation will be identified at each inspection
and removed.
Spill Prevention Measures shall be exercised on site when handling substances that
contaminate stormwater. Releases of pollutants shall be corrected as soon as identified.
Non -Chemical Pest Control measures shall be taken to prevent development of insects,
mosquitoes, and rodents.
