RE: Additional Street Address Assignment
Dear Property Ovrner:
I am writing in response to your request for an additional address to be assigned to the most northerly
office located on the property commonly known as 680 36ft Street, Springfield, Oregon. The property
currently has one structure with two public entrances and is located on the parcel known as Lane County
Reference Number l7O23l2L, Tax lot 02900.
Therefore, this is your notice that the most northerly office located on the above referenced parcel will
now be commonly known as 682 36e Street, Springfield, Oregon. At your request, this address
assignment takes effect immediately. The existing southerly office will remain commonly known as 680
36e Street, Springfield, Oregon.

copy ofthis letter:
