Substantial compliance with the rules of the State of Oregon for public spa
pools is indicated by the p1ans, and should be achieved if construction
adheres to the Plans.
Please be advised that the enclosed Construction Permit is permission to
build the spa pool in accordance with Oregon State Health Division Rules ' IT
IS NOT A BUILDTNG PERMIT! All necessary permits required by the city of
Springfield must be obtained from that municipality'
Following completion of construction, a final inspection is required' Please
contact me at telephone 687-3636 when that inspection is needed' An
application for an annual license and an inspection of operation by the Area
Sanitarian will also be required before permission to operate is granted'

Final inspection of the construction of the spa pool located atFountain court Apartments, 1725 N. 5th street, springfierd, oR, wasmade on June LL, L992. The inspection showed substantialcompliance with the rures of the state of oregon for public spapools.
Approval to use the spa pool for public purposes is granted,provided that all requirements of the public spa pool license aremet. A license inspection for the facility will be conducted inthe near future by you Area Sanitarian to confirm that you areneeting those operational requirements.
