Dear Ms. Daley:
Due to a subdivision being constructed adjacent to your property, the access to your property has changed
from South 32nd Street to Osage Street. Because ofthis change, it is necessary to change your street
address in order to provide the most efficient and quick response time for emergency services.
Your property located on Lane County Reference Number 18020621, Tax Lot Number 05900, commonly
known as 961 South 32nd Street, Springfield, Oregon has been changed to 3370 Osage Street, Springfield,
Oregon.
I willnotify the following companies and/or agencies of this new address by sending them a copy of this
letter:

U.S. Post Office
U.S. West Communications
Northwest Natural Gas

United Parcel Service
This change will become effective 30 days from the date of this letter (April 23, 1998). Please place your
new street address numbers on your residence and mailbox by that date. AIso, I have spoken with Dave
White of the Springfield Post Office and he said that he will deliver your mail that has been addressed
either on South 32nd Street or Osage Street through this 30 day period.
