Box 1481 Springfield, Oregon 97 477 RE: 700 l't Street, Apartments #11 and #19, Springfield, Oregon Dear Mr. Novack: On 412812006, I was requested to inspect t he above mentioned units regarding possible electrical hazards. My inspection revealed the following safety hazards. Unit # 11 o The upstairs bedroom has a broken 110 volt outlet. This must be replaced o The bathroom light has a loose connection that needs repair T o The bathroom fan is missing. A bathroom fan must be installed o The kitchen has a counter outlet to the left of the stove that has reversed polarity This must be repaired Unit #19o The Bathroom fan is missing with open wiring hanging from ceiling. The bathroom fan must be replaced and the open wiring repaired.
