I will noti$ the following companies and/or agencies of this new address assignment by sending them a
copy ofthis letter:
I am writing in response to your request for an additional address to be assigued to the most northerly
office located on the property commonly known as 680 36e Street, Springfield, Oregon. The propedy
currently has one structure with two public entrances and is located on the parcel known as Lane County
Reference Number 17023121, Tax Lot 02900.
Therefore, this is your notice that the most northerly office located on the above referenced parcel will
now be commonly known as 682 36h Street, Springfield, Oregon. At your request, this address
assignment takes effect immediately. The existing southerly office will remain commonly known as 680
36e Street, Springfield, Oregon.
