Dear Kelsy,
We recently discovered an error in the building permit fees for the recently issued permit at the Broadbase site.
Due to a technical oversight, the building permit fee was calculated incorrectly by the computer. The construction
value for the subject building project was not properly entered into the computer, so the building permit fee just
duplicated the fee for the foundation permit that was issued previously. To correct this error, additional building
permit fees still need to be collected. The following is a synopsis of the error, and the changes necessary to
correct the situation:
Foundation permit fee (based on $200,000 valuation) issued 3/9/06 $8.9!.65
Corrected building permit fee (based on $1,880,000 valuation)
Building permit fee issued 3/31/06 (based on same valuation as Foundation permit)

Conditions of Approval
1 . Special Inspection shall be provided as identified in section 1704 of the Oregon
Structural Specialty Code (OSSC) and as noted on the on the Springfield Special
Inspection Program form by the Architect of Record.
2. Site preparation shall be in accordance with the Geotechnical Engineering
Services and Seismic Site Hazard Report prepared by Professional Service
Industries (PSI) dated January 14,2002.
3. Site preparation will be performed during Wet Weather Conditions and precaution
shall be taken to avoid all unnecessary disturbance or tracking of moisture-
sensitive soils.
4. Tracking of soils or debris unto City rights-of-way is prohibited.
5. Upon completion of the placement and compaction of the structural fill and all
soil excavations, a complete summary and Final Report shall be provided to the
City of Springfield for review and approval per Design Review/Construction
Observation/Testing and in accordance with section 1802 of the OSSC.
6. Prior to City approval for the placement of the building foundation, a letter shall
be provided to the City from the Architect of Record stating that the building pad,
in accordance with the information provided, is acceptable for the proposed
building design.

STORNI \YATER:
Storm water from parking and landscape areas should be ror"rted to discharge to an infiltr-ation
system that incorporates vegetative treatment methods. These pians need Lo be approved by the
City of Springfield. Bio-swales are an excelient way to retard the infiltration of ninoff. Soiis
lining the swale(s) should be amended with clay as a means of funher reducing their
pen:reabiiity. A sub-surface layer of heavy clay soils beneath the pianting bedi would also serve
the pr-rpose of directing excess moisture containing fertilizers and/or pesticides from porous
planting media into the bio-swale(s).
Catch basins at loading docks (if applicabie) should be connected to the sanitary server. Areas
directly over loading docks and the catch basins should be covered. .Cut-off trenches should be
instalied r-rp-gradient to the catch basins to divert rainwater from entering the sanitary selver.
All injection wells (if applicable) must be approved by both the Cily of Springfield and DEQ
based on proximity to domestic/public drinking water wells, soils type, and depth to groundwater.
Because storm water recharges the aquifer, SIIB requests that wellhead protection signs be placed
adjacent to loading docks, trash containers and elsewhere on the site to alert contractors,
subcontractors, employees, and others to the importance of reporting and cleaning up any spills
(sign standard atrached).

I too was concerned with the 'lack' of notes related to Drinking Water Protection on the plan set. I placed this note in the
"special Condition of Approval" for the LDAP...it should help a little.
l2.This property is within the Maia Well 10-20 year time-of-travel zone (TOTZ).
o Care shall be taken to ensure that chemicals used during construction will not enter the soil or be
washed into the storm water system.
o No DNAPLs shall be used or stored on the subject site during the construction phase.
o All chemical spills or leaks shall be cleaned up immediately.
o Contractors and developers shall be responsible for the safe handling and storage of chemicals,
petroleum products, fertilizers, and the prevention of groundwater and storm water runoff
contamination. The contractor and all subcontractors are required to provide the engineer of reco
with copies of MSDS sheets for all chemicals proposed for use on-site prior to use or delivery to the
property. Based on these MSDS sheets, the engineer of record will determine if the chemical is
approved for use on this site. A copy of the MSDS sheets for all chemicals used on the project will
remain on the job site as required by law.
o All chemicals shall be placed in adequate secondary containment.

l'm reviewing a Site Plan Modifiaction application on this one today. I haven't finished the review yet. I'm assuming the
structuralfill is for the building only, as we have not approved the site plan modifications yet.
My only concern is that this property is located within Springfield's Drinking Water Protection Overlay District and is subject
to SDC Article 17. There is no reference in the applicant's plans to the DWP Overlay. Specifically, the propefty is within
the l0-20 year Time of Travel Zone (TOTZ) for the Maia Well. Special protections are necessary at this development to
Protect drinking water. Construction practices have potential to pollute groundwater.
