I am writing to you in regards to electrical permit # coM2006'0024l' This permit will
expire on September 1, i006. We recently realized tlat y: will be unable to finish the
electrical work on our residence by the s"pt".u"r l_ deadline. The inspector has already
inspected our work twice and the onfy pu.i of the job that is left is dry walling the soffets
and patching up some holes in or. *uti, in order io pu.t the final inspection' We are
concerned about tt"-q"Jity of our work and want to do it right' After-we began the dry
wall work in one of our rooms, we realized that the job is more work than we expected'
In order to complet"ltl nnuf steps of the job, we lt"q.u little more time' Therefore' we
are requesting an extension for our electrical permit. Please contact me at 747-3089 and

licensed with the Construction Contraitors Board to sign the following statement before a building
permit can be issued. This statement ts requiredfor residential building, electrical, mechanical and

If you are acting as your own contractor to construct a new home or make a substantial improvement to an existing
structure, you can prevent many problans by beirry aware of the following responsibilities and concerns.
Employer Responsibilities
You will, in most instances, be ruled to be an "employer" and the cqntractors you conkact with will be .'employees" if
you us9 contractors not licensed with the Constuction Conhactors Board to do labor in constructing or to assist in the
construction or improvement of a residential struchre. ^A.s the employer, you must comply with th'e following:
Oregon's Withholding Tax Law: As an employer, you must withhold income taxes from employee wages at the time
ernployees are paid. You will be liable for the tax payments even if you don't actuatty wittrfrota the tax from yor:r
employees. For more information,'call the Dryartmort of Revenue at 503-3?84988.
Unemployment fnsurance Tax: As an employer, you are required to pay a tax for unomployment insurance purpor&
on the wages of all employees. For more information, call the Oregcn Employment Departmen t at 5A3-g47- f +AS.
The Oregon Business Identification Number (BS.I) is a combined number for both Oregon Withholding and
Unemployment Insuranse Tax. To file for a BIN, call 503-945-8091 or www.dor.state.or.uslformspay.htmll for the

Workers' Compensation Insurance: As an employer, you are subject to the Oregon Workers' Compensafion Law,
and must obtain workers' compensation insurance for your employees" If you fail to obtain workersi compensation

coverage for accidents and omissio,ns such as falling tools, paint over spray, water damage from pip punctures, firs or
work that must be redone.
