D. As noted on the CAP, the label must be delivered to the

supervisor, The general contractor is not authorized
to sigh the electrical label.
Applicant to sign and date
whenever possib'l e, the initial application will be used as
a worksheet on1y. Where possib'le, Building Division Staff
will prepare a type written copy and return it to the
applitant at the time the actual permit is issued for his
s i gna ture.
Fees and Charges
P'lan check feis are due and payable at the time of the
application, and no plans wi)1 be processed until these
fees are paid. All other fees and charges are due and
payable when the Permit is issued.

3. C0NDITI0NS. The Appl icant agrees to fulfill all specif ic cond'it'ions ofapp19vq1 required by the C'ity ljsted jn RECITALS prior to occupancy, unless certaincondjtions have been deferred to a later date in accordance with-Sectjon 31.040(4)(b)of the Springfield Development Code.
4. M0DIFICATI0NS. The applicant agrees not to modify the approved Final Site
Plan wjthout first notifying the City. Mod'ifications to the Final S'ite Plan shall be
reviewed in accordance with Sect'ion 31.100 of the Spningfield Development Code.
5. MAINTAINING THE USE. The app'licant agrees to the following:
(a) The building and site shall be maintained jn accordance with theprovisions of the Springfield Development Code in order to continue the use.

Occupancy Permjt which may have been granted.
7. Any Final Site Plan approved becomes null and void if constructjon does not
commence wjthjn one year of the date of th'is Agreement.
