The inspection took place at 1 1 : 1 5 u.* on November 8,2002. Weather was cloudy and rainy with an outside air ternperature of approximately 45 degrees F. The work area was covered and protected from direct rainfall. The3l| inch diameter hole rl- drilled approximately 11 ll2 inches decp and blown and brushed clean. The edge distance to the outside face of the concrete stem wall was approximately 2ll4 inches. \ The hole was then filled with epoxy ("Fisher" FIS VS 150C adhesive with 3 6iai6rrm bas€ material temperature requirement of 32 degrees F), the 5/8 inch diameter rod inserted and plunged to remove air pockets, and then seatd futly into the hole, for an embedment of approximately 11 inches.
