Note: Oregon Law, ORS 701.055(4) requires residential construction permit applicants who are not
licensed with the Construction Contractors Board to sign the following statement before a building
permit can be issued. This statement is requiredfor residential building, electrtcal, mechanical and
plumbing permits. Licensed architect and engineer applicants, exempt from licensing under
ORS 701.010(7), need not submit this statement. This statement will befiledwith the permit.

RE: Inspection of epoxied anchor rod for above referenced residence
Branch Engineering,Inc performed an inspection of the installation of a single threaded rod
epoxied inio an exiiting ioncrete stem wall at the residence referenced above. The inspection
took place at 1 1 : 1 5 u.* on November 8,2002. Weather was cloudy and rainy with an outside
air ternperature of approximately 45 degrees F. The work area was covered and protected from
direct rainfall.
The3l| inch diameter hole rl- drilled approximately 11 ll2 inches decp and blown and brushed
clean. The edge distance to the outside face of the concrete stem wall was approximately 2ll4
inches. \
The hole was then filled with epoxy ("Fisher" FIS VS 150C adhesive with 3 6iai6rrm bas€
material temperature requirement of 32 degrees F), the 5/8 inch diameter rod inserted and
plunged to remove air pockets, and then seatd futly into the hole, for an embedment of
approximately 11 inches. The inspection was concluded at this stage-

The3l4 inch diameter hole was drilled approximately 16 inches deep and blown and brushed
clean. The edge distance to the outside face of the concrete stem wall was approximately 2
inches.
The hole was then filled with epoxy ("Simpson" SET-PAC adhesive), the rod inserted and
plunged to remove air pockets, and then seated fully into the holes, for an embedment of
approximately 16 inches- The inspection was concluded at this stage.
