hereby certify that all information herein is true and correct, and I further certify that any and all work
performed shall be done in accordance with the Ordinances of the City of Springfield, and the Laws of
the State of Oregon pertaining to the work described herein. I also agree to call for the required
inspections as noted above (726-3769) at the appropriate times. I also state that I was provided with
Lane Regional Air Pollutions phone number and asbestos removal information. I further agree that
the project address will be readable from the street, and the permit card is located at the front of the

340-07I-0205 AUTHORIZATION TO USE EXISTING SYSTEMS
(l) Authorization Notice Required. Except as otherwise allowed in this division no person
shall place into service, re-connect to, change the use of, or increase the projected daily
sewage flow into an "*iiling oi*ile sewage disposal system without first obtaining an
Authorization Notice, ConJruction-lnstallation Permit or Alteration Permit as appropriate
EXCEPTIONS:

An Authorization Notice is not required when a mobile home is replaced with
similar mobile home in a mobile home park, or a recreation vehicle is replaced
by another recreation vehicle in a lawful recreation vehicle park, provided the
sanitary wastewater system has adequate capacity for safe treatment and disposal
of sewage generated within the park;
An Authorization Notice is not required for placing into service a previously
unused system for which a Certificate of Satisfactory Completion has been
issued within five (5) years of the date such:system is placed into service,
L providing the projecred daily sewage flow does not exceed the design flow, and
there is no other violation of these rules.

An application for the Authorization Notice shall be submitted on a form approved by
the Department. The application is contplete only when the form, on its face, is com-
pleted in full, is signed Ly tne owner or the owner's legally authorized representative' and
i. u..o*punied by all required exhibits and fee. The exhibits shall include:
(a) A land use compatibitity statement from the appropriate land use authority
signifying that the proposed land use is compatible with tlre Land Conservation
and Development Commission acknowtedged comprehensive plan or complies
with the statewide Planning goals:
An accurate property development plan:
A sewage treatment and disposal system description;
Tax lot map or equivalent plat map for the property:
Documentation of hardship if such is being claimed:

For placing into service or for changes in the use of an existing on-site sewage disposal
system *here no increase in sewage flow is projected. or where the design flow is not
exceeded; an Authorization Notice valid for a period not to exceed one ( l) year may be
issued if:
(a) The existing system is not failing; and
(b) AII set-backs between the existing system and the structure can be maintained;
and
(c) In the opinion of the Agent the proposed use would not create a public health
hazard on the ground surface or in surface public waters'

MEMORANDUM TO ADDRESS FILE
A single wide manufactured home permit is issued to this location, as per the following
chronology:
Applicant, Robert W. Smith, was required to address a nuisance abatement order from
DEQ for a failed septic system. Property could not be served by sanitary sewer.
A single family house and single wide manufactured home were located on the property.
The house was demolished and the manufacture home removed.
The Development Code (SDC 5.030(5)) allows a single wide to be replaced on the lot,
which would otherwise be unlawful, provided the replacement is completed within 90
days.
The single wide manufactured home was not replaced within 90 days.
The applicant is granted the grace period to allow the replacement of the single wide for
the following reasons:
l. The City did not have jurisdiction over the agencies empowered with approving a
replacement septic system on this parcel.
2. Lane County and DEQ found that a standard replacement system would not work
and required the applicant to engineer a unique alternative system which took six
months to engineer, design and the receive county and state approval.
3. The approval was granted during the winter and the system, by its design, could
not be installed until the soil was dry.
The applicant installed the system in the late spring, 2002 and submitted for a
manufactured home installation permit.
