OAR Chapter 340, Division 7, Section 72-02016l: This report shall remain'in efiect until issuance of a
permit to construct, unless in the meantime conditions on this or adjacent properties have been altered in
any manner which would prohibit issuance of a permit, in which case this evaluation report shall be con-
sidered null and void. Technical rule changes will not invalidate any evaluation report issued pursuant to
this section.
Before construction is started bring this form to the Department of Environmental Management and apply
for a building permit. Exact specifications for the sewage disposal system will appear on the permit.
lf the property is only a portion of a tax lot, a partitioning or subdivision must be approved before the
permit will be issued.
Only persons.having a valid license issued by the Department of Environmental Ouality under ORS 454.695
to install subsurface sewage disposal systems or an owneror contract purchaser or his regular employees
may install subsurface sewage disposal systems. A certificate of satisfactory completion will not be issued
if persons other than the above perform the installation.

The following rules are established under authority of that order.
(a) One sign shall be posted for each 300 feet, or portion thereof, of foot-
age on any public street or road. If the property abuts more than onestreet or road, at least one sign sha1l be displayed along each abuttingstreet or road.
(b) If no public street or road abuts the property, signs shall be displayed
in such a manner as to be most noticeable to the public. The manner ofposting shal1 be determined by a member of the Planning Division and given
to the applicant in writing.
Signs will be prepared by the Lane County Planning Division and provided to the
applicant.
Signs will be displayed i.n clear view in such a manner as to be visible to an
automobile traveling on the abutEing street or road at a normal rate of speed.
The signs sha1l be readable by a person standing at the edge of the street or road.
4 Signs shall not be displayed in such a way as to obstruct views or cause traffic
hazard.
5. The signs sha1l be displayed at least 10 da;rs but no more than 15 days prior
to the date of the hearing as shown on the sign. The signs shall remain ondisplayuntil after the hearing date and removed within 7 days of that date.
Placing and removal of the signs is the responsibility of the applicant.
6. If a sign is displayed on property not under the orrmership of the applicant,permission to post such signs should be obtained from the property o\^rner.
7 - Fail,ure to comply with these requirements will result in the cancellation ofthe schecltrled hearing. Rescheduling the hearing will require payment of ad-ditional fee of 925.00.
8. The applicant wi"11 not be held responsible for removal, loss or damage of thesigns through vandelism or similar acts. The applicant should notify theLane County Planning Division if such problem occurs.
9. The applicant must sign a receipt for the signs and an agreement to complywith these requirements.
