Dear Mr. Lee:
By request of the Springfield Fire Department an inspection of the electrical
wirind of equipment in lhe bar, dance and lounge areas of the Hacienda Steak
Hour.-tocatdd it aSSO Gateway Street, Springfield,0regon has revealed the fol-
ioring tiited defects to be Lorected. 'These corrections must be completed by
AO Oais from the date of this letter (August 13' 1982).
, L) Defective receptacle by the rear exit door to be repaired and cover p'late
installed. E.S.S.C. 370-15
Z) Remove cords across walking areas and install approved pendant cords from
Ceiling outlets to supply iemporary island equipment. E.S.S.C. 400-8
3) All permanent'ly installed (cord connected) electricai equipment shall have' receptacles initalled adjacent to such equipmelt.to eliminate the use of
extehsion cords. Conduit to be the wiring method. E.S.S.C. 518-3 & 400-8
4) All electrical equipment w'ithin reach of personnel shall be grounded.
Cords shall contain a grounding conductor. E.S.S.C. 250-42 & 250-45
5) Surface metallic raceway is an approve{ qnd recornmended method of instal-
Iing extension receptacles. E.S.S.C. 35?'L

tt is our practice to notify businesses when we have notice, or have received a complaint regarding a
possible violation of the Sign Code on your property.
Banners are allowed by a special permit once a year for a ten (10) day period. The cost of the permit is
$40.00 with a refundable $100.00 deposit provided the banners are removed by the end of the tenth day. I
have enclosed the Code section and an application for your use ifyou choose to apply. The banner that is
currently located on your property needs to either be removed, or you can apply for the special banner
permit within the next seven (7) days.
