As requested by John Hyland Construction, Earth Engineers, lnc. representative Pete Furlong was at the above
referenced project site on this date to perform periodic inspection on Simpson Strong-Tie Titen HD Screw Anchors
installed at Building 5 in sill plate to concrete slab on grade in suites 830, 840 and 850.
The seventy-five (75) 5/8" x 6" Simpson Strong-Tie Titen HD Screw Anchors were installed in accordance with the
Typical Sill Plate Attachment Detail 1/S4.3 in the City of Springfield approved project plans. Note: the screw anchors
had already been installed and tightened by the contractor prior to our arrival on site. ln accordance with Table #1 in
the ICC ESR-2713, the maximum installation torque of 1ooft-lb was verified on 100% ofthe 5/8" anchors. The torque
was verified using our Craftsman Microtork Torque Wrench (Model No. 34576) with a calibration date of May 30,
2018
To the best of our knowledge, the work inspected on this date by EEI and noted above was in conformance wilh the
City of Springfield approved jobsite plans, manufacturer's installation instructions for the Simpson Strong-Tie Titen HD
Screw Anchors, ICC ESR 2713 and applicable provisions of the lBC.

As requested by John Hyland Construction, Earth Engineers, lnc. representative Pete Furlong was at the above
referenced project site on this date to perform periodic inspection on Simpson Skong-Tie Titen HD Screw Anchors
installed at Building 4 sill plate to concrete slab on grade in Suite 600.
The 5i8' diameter x 6' long Simpson Strong-Tie Titen HD Screw Anchors were installed in accordance with the
Typical Sill Plate Attachment Detail 1/S4.3 in the City of Springfield approved project plans. Note: the screw anchors
had already been installed and tightened by the contractor prior to our arrival on site. ln accordance with Table #1 in
the ICC ESR-2713, the maximum installation torque of 1ooft-lb was verified on 95% of the 5/8" diameter anchors. The
torque was verified using our Craftsman Microtork Torque Wrench (Model No. 37425) with a calibration date of May
2018.
To the best of our knowledge, the work inspected on this date by EEI and noted above was in conformance with the
City of Springfield approved jobsite plans, manufaclurer's installation instructions for the Simpson Strong-Tie Titen HD
Screw Anchors, ICC ESR 2713 and applicable provisions of the lBC.

As requested by John Hyland Construction, Earth Engineers, lnc. representative Pete Furlong was at the above
referenced project site this date to perform continuous inspection on 4325 high strength bolts (HSB) located in
Building #4:
On this date we inspected lhe 112" x2-112" A325 HSB and 314" x2" A325 HSB that were installed between gridlines A
and N from 1 to 2. Using the conlractor's Williams HTW 4CP7 torque wrench (S/N 433602), that was calibrated on
May 31 , 201 8 (see EEI report 18-013-79), at least 80% of the HSB were inspected and found to be torqued to 1 50 ft.-
lbs. for the 1/2" HSB and 300 ft.-lbs. for the 3/4" HSB.
To the best of our knowledge, the work inspected by EEI on this date and described herein was in accordance with the
City of Springfield approved project plans, the City of Springfield Varco Pruden drawings, and the applicable provisions
of the IBC and AISC.

All-Thread Epoxied Anchor Bolts: continuous inspection was performed during the installation of one hundred thirty
two (132) 3/4" all-thread epoxied anchor bolts in the following column base plate locations: B at 2, B at 3, B at 4, C at
2,Cat3,Cal4,Dal2,Dat3,Dat4,Eat2,Eat3,Eat4,Fat2,Fat3,Fat4,Gal2,Gat3,Gat4,Hat2,Hat3,
H at4, J at 2, J at 3, J at4,Kat2, K at 3, K at 4, L at 2, L at 3, L at4, M at 2, M at 3 and M at 4. Priorto installation,
the pre-drilled holes were cleaned with compressed air and a nylon "bottle" brush. The above listed anchors were
installed using Hilti Hit-RE-5000 V3 epoxy with an expiration date of 9-2-2019 and an ICC ESR-3814. The holes were
verified to be the proper embedment depth of 1 l'. Prior to installation, the contractor discarded several pumps of the
initial epoxy to ensure both parts were adequately mixed. Contractor stated the nuts would be tightened after the
epoxy has cured. Contractor was asked to notify our office when additional services are needed.

Welds: periodic inspection was performed on welds at the embed plates to tilt up panel connections in the following
localions: Tilt-up panel to footing connection in accordance detail 1/S4.2 (Typical to Foundation Connection); tilt-up
panel to tilt up panel connection in accordance with detail 2154.2 (lypical Panel Alignment Clip Detail); and tilt-up
panel to tilt up panel connection in accordance with detail 5/54.2 (Typical lnside Corner Connection). All welds were
inspected for size, length, profile, and visual discontinuities including porosity, cracking, and incomplete fusion. The
welds were not painted at the time of our inspection. All welding was performed by Nicholas Cunningham with Rock
Hill Welding and Fabrication. A copy of Mr. Cunningham's welding certificate was provided to us and will be kept in the
project file.

Simpson Strong-Tie Titen HD Screw Anchors, model no. THD75400H, installed along gridline N between 1 and 5 in
accordance with detail 1'1lS4.2. Note that the screw anchors had already been installed and tightened by the
contractor prior to our arrival on site. ln accordance with Table #1 in the ICC ESR-2713, the maximum installation
torque of 15oft-lb was verifled using the contractors Armstrong Torque Wrench #'16052930.

As requested by John Hyland Construction, Earth Engineers, lnc. (EEl) representative Pete Furlong responded to the
site on this date to perform continuous visual inspection during the placement of concrete at Building 5 loading dock
walls.
Note: reinforcing steel for the above referenced area was inspected on August 23, 2018 (reference our report 18-01 3-
e1).
Our representative performed continuous inspection during the placement of concrete in the above referenced
locations. Approximately 21.50 yds3 of Knife Rivers approved mix design no. 24V0N55C00 (4,000 psi, 3/4" aggregate,
no air) was deposited by boom pump and mechanically consolidated throughout its placement depth. Concrete trucks
were inspected for mix design, batch time, slump, and temperature. For more detailed information on concrete placed
on this date please reference our concrete strength test report 1 8-013-93.
Our representative also cast one set of four (4" x 8') concrete test cylinders from a sample obtained from the 1"t
concrete truck load. Our sample had a measured slump of 7". The test cylinders were stored on site in a cooler with
temperature-conditioned water for initial curing in accordance with ASTM C31. The test cylinders will be transported to
our facility on August 29, 20'18 and placed in a concrete curing tank (lime and water solution) for final curing. The
cylinders are scheduled to be tested for compressive strength in accordance with ASTM C39 at (1) 7-day and (3) 28-
days. For more detailed information on concrete placed on this date please reference our concrete strength report 18-
013-93.
To the best of our knowledge, the work inspected by EEI on this date and described herein was in accordance with the
city of Springfield approved project plans, approved mix design, and the applicable provisions of the lBc.

Earth Engineers, lnc. (EEl) representative Pete Furlong was on site this date to pick up 1 set of 4 (4" x 8") concrete
test samples that were cast on August 28, 20'18 (reference our report 1 8-013-92). The samples were transported to
our facility for final storage in accordance with ASTM C31 procedures and compression testing in accordance with
ASTM C39 procedures. The results of the tests will be presented in our report 1 8-013-93.

As requested by John Hyland Construction, Earth Engineers, lnc. (EEl) representative Ray Aliperti was on site this
date to perform periodic visual inspection of reinforcing steel for the Building 5 loading dock walls.
The reinforcing steel was inspected for grade, size, Iap, placement and clearances where applicable. The rebar was
secured against displacement using annealed tie wire. The reinforcing steel was placed in accordance with the revised
plan sheet and details on S4.5 for the proposed Building 5 loading dock.
To the best of our knowledge, the work inspected by EEI on this date and described herein was in accordance with the
City of Springfield approved project plans, revised plan sheet S4.5, and the applicable provisions of the lBC.

As requested by Stephen Klemmer with John Hyland Construction, Earth Engineers, lnc. representative Pete Furlong
was at the above referenced project site on this date to perform inspection on 4325 high strength bolts (HSB) in the
beam to beam and beam to column connections located in Building #5.
On this date we inspected all the 4325 HSB that were installed in the beam to beam and beam to column connections
from gridtine A to N between 1 and 3. Using the contractor's Williams HTW 4CP7 torque wrench (S/N A33602), that
was calibrated on May 31, 2018 (see EEI report 18-013-79) we inspected at least 80% of the HSB. The 1/2" HSB were
verified to be torqued to 150 ft.lbs. and the 3/4" HSB were verilled to be torqued to 300 ft -lbs'
The A32S HSB located at the Lateral Frame Column along gridline G from 1 to 3 were tensioned using Turn-of-the-Nut
method. The 4325 HSB were initially verified to be snug{ight then the bolts were match marked and then the
additionat 1/3 turn applied as required in the AISC manual.
To the best of our knowledge, the work inspected by EEI on this date and described herein was in accordance with the
city of springfield approved project plans, lcc ESR-3814 and the appticable provisions of the lBc and Alsc manual

As requested by John Hyland Construction, Earth Engineers, lnc. (EEl) representative Ray Aliperti was on site today
to observe a proofroll at finished subgrade elevation under the proposed Building 5 loading dock.
The proofroll was performed to assist us in determining the stability of the exposed soils which appeared to be a
mixture of 3/4"-minus, 1-1l2"-minus, and well-graded gravel with silt and sand (tocally referred to as "bar-run" gravel).
Prior to performing the proofroll, the surface of the area was compacted using a large vibratory Smooth drum roller'
The proofroll was conducted using a loaded tandem axle water truck weighing approximately 40,000 pounds No
deflections (pumping) were observed under the wheel loads of the dump truck and no wheel tracking (rutting)
occurred, indicating a well-compacted and stable surface at this time'
To the best of our knowledge, the work observed on this date by EEI and noted above was in accordance the city of
Springtield approved plans and applicable provisions of the IBC'
