Performed preliminary reinforcing steel inspection for foundation mat slab on west side
of building. Reinforcing steel was checked for grade, size, lap, placement, quantities
and clearance, where applicable. Final inspection will be performed prior to concrete
placement.
To the best of our knowledge, the work inspected was in accordance with volume 5
plans daled 05/2018 and applicable workmanship provisions of the LB.C., excepl if
noted above.

Performed inspection on the reinforcing steel for the sump base near lines 1.58.
Reinforcing steel was checked foI size, grade, laps, spacing, quantity, ties, supports
and clearances, where applicable. Also, verified waterstop was installed and secured.
Observed the placement of 1.25 yards of RiverBend Materials 5000 psi concrete (mix
390580). Concrete was placed with a track-hoe and was mechanically consolidated.
Cast one (1) set of four (4) 4" x 8" concrete test cylinders. Slump - 3 3/4"; concrete
temperature - 58F; air temperalure - 44F; air content - 4.5olo.
Ken Meyer (PacEx) and Mike Stickling (EWEB) were advised of the on-site tesl results.
To the best of our knowledge, the work inspected was in accordance with volume 5
plans dated 05/2018 and applicable workmanship provisions of the l.B.C., except if
noted above.
