Performed continuous inspection during the installation of six (6) 1/2" x 5 1/2" vertical
Hilti KB-TZ CS wedge anchors (lCC - ESR 1917) securing patient table base and four
(4) 3/8' x 3 3/4" vertical Hilti KB-TZ CS anchors securing generator cabinet to 'l si floor
slab in room 'lQ21. Predrilled holes appeared relatively clean and free of excessive
drilling dust. Verified minimum embedment depth ol 3114" tot 1/2" anchors a d2" fot
3/8" anchors. Afler installation, verifjed installation torque of 40 fvlbs. for 1/2" anchors
and 25 fulbs. for 3/8" anchors with a calibrated torque wrench.
To the best of our knowledge, the work inspected was in accordance with KPFF sketch
SSK-1 dated 06-15-18 and applicable workmanship provisions of the l.B.C., except if
noted above.

Performed periodic inspection during the installataon of fony-two (42) 112" x 7' vedical
Hilti KB-TZ wedge anchors securing Unistrut P1000 brackets and braces to underside
of 2d floor slab in X-Ray #6 room '1Q21. Site visits were made on September 26, 28 &
October 06.
Predrilled holes were cleaned during drilling with a Hilti drill/vacuum system. Verified
minimum embedment depth of 3 1/4". Anchors were installed with a Hilti impact driver
and tightened to 40 fl.Ibs. Verified installation torque \ryith FEI'S calibrated torque
wrench.
Kelly Willis (GBC) was asked lo notify our office when additional services are needed.
To the best of our knowledge, the work inspected was in accordance with NW CeilingLifts submittal dated 09-07-18 and applicable workmanship provisions of the l.B.C_
except if noted above.
