and final visual inspection for columns 39A & 398. lnspected fit-up and monitored amps, volts and
travel speed of one ('l ) certified welder using the FCAW welding process E71T-1 filler metal.
Compleled welds were checked for size, length, profile and location.
R€marks: Structural drawings do not contain weld details for column connections. Columns were
fabricated per attached shop detail.
To the best ot our knowledge, the work checked is in conformance with applicable AWS Dl.1 code,
except if noted above.

Performed periodic inspections during the installation of approximately five hundred
and two (502) 5/8" x 8' Simpson Titen HD screw anchors into concrete slab for Type 'l
& 3 shear walls and load bearing wall sill connections from line A to E, between lines '1
& 7 in the easl, west and south wings. The holes were cleaned during drilling using a
Hilti drill/vacuum system. Anchors were installed with 2" x 2" plate washers using a
Milwaukee '110V electric impact wrench to snug{ight condition. Site visits were made
on December 07, 11 & 18.
Tim Jacobs (CCC) was asked to notify our otfice when additional services are needed
To the best of our knowledge, the work inspected was in accordance with structural
drawings dated 02-06-'18, RFI 70 and applicable workmanship provisions of the l.B.C.,
except if noted above.

Performed continuous inspection of complete ioint penetration groove welds on HSS rigid frame
connections at lines 6C.5 and HSS beam to HSS column connections along line O.5, between lines 5
& 6. lnspected fit-up and monitored amps, volts and travel speed of two (2) cerlified welders using the
SMAW welding process E7018 and FCAW welding process E71T-8 filler metal. Completed welds
were checked for size, length, profile and location.
To the best of our knowledge, the work in-progress is in conformance with structural drawings dated
03{8-18 and applicable AWS D1.1 code, except if noted above.

Performed continuous visual inspection on parlial joint penetration groove welds for HSS beam to
HSS beam connections on sun shade at lines 5.5E. lnspected fit-up and monilored amps, volts and
travel speed ot one (1) certified welder using the FCAW welding process E71T-8 and SMAW E7018
filler metal.
While on-site, performed visual weld inspection on continuous corner angle to HSS connections for
,rames at west lounge 109 at 3A.5 and east lounge at 6C.5. Completed welds were checked for size,
length, profile and location.
Note: Continuous angle to HSS connections were welded per conversalion between Tim Jacobs
(Chambers Construction) and EOR in lieu of all around weld due to joinl configuration.
Items requlring completion: Continuous angle to HSS connections do not mo€t accaptance
criteria for AWS D1.1 code due to ercessiye porosity, slag inclusions and lack of fuslon for
€ast lounge t 09.
Mr. Jacobs was asked to nolify our office when additional services are needed.

06-18, Kiwi Fab drawings approval date 08-06-18 and applicable AWS D1.1 code, excepl if noted
above.
