Dear Mr. Cassidy:
On July 7, 1983 a Fire Safety Inspection was made by the Springfield Fire Department.
Obvious electrical hazards noted during this inspection were referred to the Building
Safety Division. On July 11, 1983 a correction notice was left at the inspection site
to make corrections within 30 days. On August 11, 1983, a follow up inspection was
made. At this time it was found that the corrections required on July 11, 1983 had
not been done nor had an electrical contractor been consulted. It was agreed in dis-
cussion that work would'be started in a week (7' days). On August 19, 1983, another
follow up inspection was made, revealing that corrections still had not been made.
The 'attached corrections:required on July 11, 1983 are listed for your convenience.
These corrections must be completed within fifteen (15) days of the date of this let-
.ter, or it will be necessary to refer to the matter to the City Attorney's office for
further action. '

CORRECTIONS REQUIRED
1. Remove cord wiring used in lieu of permanent wiring in all areas..
(a) Conduit supplied outlets shall be installed for all equipment that requires
flexibility adjacent to the equipment.
(b) All receptacle outlets shall be of the grounding type.
(c) Cords to equipment in grounded areas shall contain a grounding conductor
unless equipment is of double insulated construction.

.(d) Stationary equipment. shall be supplied directly with conduit where flexi-
bility is not required.
2. All boxes (enclosures containing wiring) shall have covers installed and unused
conduit holes closed with approved seals.
3. Clearly' identify all circuits/breakers in the service panel.
4. Secure loose conduits in the printing room.
5. An Electrical Contractor shall perform all work on the listed defects.
6. Corrections are to be made within 30 days of the date of notice. '
