December 3, 1981 / Mr. Al Cook 630 Main Street Springfield, Oregon 97477 * RE: Electrical Safety Inspection at 630 ~Iain Street, Springfield, Oregon Dear Mr. Cook: In response to the request of the Fire Marshal, inspection was made of the electrical wiring and equipment in the Fine Arts Theatre, 630 Main Street, Springfield, Oregon. The fOllowing listed violations of the Oregon Electrical Specialty Safety Code are considered hazardous to persons and property and shall be corrected by an Oregon li- censed electrical contractor within the immediate future, within 30 days (1-~-82). 1) Clearly identify each service panel, indicating use and area, in all locations in the building. Clearly identify each branch circuit within the service panels, in- dicating ~se and location. E.S.C. 110-22 2) Replace defective lamps in EXIT light fixtures. E.S.C. 700-16 3) Replace missing covers on junction boxes, control switches and thermostats in all areas. Install dead front on panel near lobby. E.S,C. 410-12 4) Seal open holes (K-O's) in junction boxes and service equipment, E.S.C. 370-8 5) All unused wiring to be disconnected at the source of supply, or properly termi- nated in splice boxes, and protected by over current devices in accordance with conductor ampacity. E.S.C. 210-20 (a) 6) Install lamps in open sockets in stage foot lights, or use approved plugs. Remove spot light type bulbs that are located close to wood low 2" clearance for all other lamps next to combustible material, and 47 lamp holder railing. Al- E.S.C. 520-6 7) Remove all fixture t)~e (Zip-cord) w1r1ng in consession area, Install conduit to receptacles installed adjacent to cord connected equipment. Equipment that requires movement or is portable .shall have S, SO, ST, or STO type cord with ground conductor and grounding t)~e cord caps. Receptacles shall be of the grounding type. E.S.C. 520-81 and 44 (b) 8) Install conduit to lighting fixtures over concession area. Conduit shall be the wiring method to all permanently installed equipment (fixed or secured as to lcca- tion). E.S.C. 370-10 and 12 10) Replace missing glass in restroom lighting fixtures. EXCEPTION -- ~~ere constant or regular movement of equipment or parts or equipment is necessary, flexible steel conduit may be used. S, SO, ST, or STO type cord may be used for portable equipment. All cord shall have grounding conductor and grounding type cord caps. All receptacles shall be of the grounding type and be located adjacent to cord connected equipment. E.S,C. 540-15 13) Electrical equipment, in general, in the projector room shall be checked by the electrical contractor for loose fittings and conduits, openK-O's, proper strap- ping and securing of equipment, proper grounding and wiring connections, conduc- tor ampacities and the required over current protection. . E.S.C. 110-3 (8) 14) All projection ports, spotlight ports, viewing ports and similar openings shall be provided. with glass or other approved material so as to completely close the opening. E.S.C. 540-10 15) Section 814-22-110 of the Oregon Electrical Specialty Safety Code states that an electrical contractor shall perform all electrical work on properties for rent, sale or lease.
