Community Services Division, Building Safety, I further certify that only
contractors and employees who are in compliance with ORS 701.055 will be
used on this project,

card is located at the front of the property, and the approved set of plans
will remain on the site at all times during construction,

The applicant agrees that if he/she does not pave the area behind the sidewalk within 30 days of cutting Ole
curb opening, the City of Springfield has the authority to close the drive\\-'3Y access by removal of the curb cut.
