Subject: Occupancy Inspection at 630 Main Street, Springfield, Oregon.

Proposed Use: Assembly Use - Private Dancing Facility
Dear Mr. Myers,
At your request, the Community Services Division/Building Safety conducted an
inspection of the building located at the above address on June 13, 1997. The purpose of
the inspection was to determine the suitability of the building for the proposed use as
indicated.
Based on the proposed occupancy, the existing conditions which are mentioned below do
not meet the minimum Building Safety Code requirements. Corrective measures must be
taken prior to occupancy to install, repair, replace or modify the following items in order
for the building to conform to applicable safety codes: '

I. All emergency exit lighting shall be operationaL
2. Currently there is a 14 inch step at the northeast exitway. The maximum allowable
step-height for an exitway is seven inches_ Stairs or a ramp must be installed in its
place.

1, The shower in the men's rest room has been installed without the required permits or
inspection approvals, Please purchase a plumbing permit and obtain inspection
approvals or remove the shower.
ELECTRICAL
I, There have been lighting fixtures and other wiring installed by the previous tenants
without permits or inspections, In the attic area there are uncovered junction boxes
and areas with exposed wiring that needs to be repaired, It is recommended that you

have a licensed electrical contractor make the needed repairs and check the integrity
of the rest of the electrical system. All electrical installations, alterations or
modifications shall conform to present Electrical Specialty Safety Code requirements.
Building permits must be obtained for the above items which involve repairs or
modifications to the structural, electrical, plwnbing or mechanical systems of the building
and for any additions or revisions you wish to make to the building.
