Field Observations and Comments
Performed continuous inspection during the installation of 3/4" all thread epoxy anchored bolts in the following locations:
32 placed on south wall top. Prior to installation the predrilied holes were cleaned using a nylon brush and compressed
air. Verified minimum embedment depth of 7 inches. The above listed epoxy anchors were installed using Hilti Hy 150
epoxy, Contractor stated that the nuts would be tightened after the epoxy has cured, The epoxy anchors were installed
in accordance with approved job site plans. Note: bolts #4 and #5 from the southeast corner had 6 1/2" embed, This
will require engineer's acceptance. To the best of our knowledge, the work inspected above was in accordance with the
building department approved design drawings, specifications, and applicable workmanship provisions of the
International Building Code. Contractor was asked to notify our office when additional services are needed. Non-
comoliance.
