Performed continuous inspection dunng installation of 5/8" all thread epoxy anchored bolts In the following locations.'
Hold down anchors at east sIde of east garage door, duplex addition Prior to installatIOn the prednlled holes were
cleaned by nylon brush and compressed air Venfled minimum embeddment depth of 6". The above listed epoxy
anchors were Installed using Simpson Strong Tie In accordance with manufacturer's directions Contractor stated
nuts would be tightened after the epoxy has cured. Contractor was asked to notify our office when additional services
are needed
