PURPOSE AND SCOPE
As requested, K & A Engineering, Inc. has provided on-site inspection of foundation underpinning of the
foundation of your home at the subject site address. This work was permitted by the City of Spnngfield and
was completed by Van Dam Construction, Inc.
Our services included on-site inspection of the piers during Installation, inspection of the leveling of the
foundation after piers were installed, and this summary report.
PIER INSTALLATION
Underpinning Design
Underpinning for this project was deSIgned by K & A Engineering, Inc. and consisted of placement of steel
helical piers to support the existing concrete foundation. The design was specifIed in a repair plan that was
part of our geotechnical foundation investigation report to you dated April 22, 2005. Our recommendations
InCluded specific locations, pier type, and minimum ultimate pier capactties to be achieved.
Pier Installation
The helical piers were installed at the locations with two exceptions. See the attached As-built Pier Plan. An
additional pier was required (pier no. 5) due to the presence of a large rock at the originally specified location.
Also, and additional pier was needed near the water heater, pier no. 17, due to an error in our original foundation
plan's dimensions.
Piers 1-5 and 10-35 consisted of 2.875-mch diameter schedule 80 high-strength steel pipe with a 10.inch, 12-
inch, and 14-inch helical plate welded to the bottom lead, separated by approximatelv 2-feet Piers 6-9
consisted of a 2.875-inch diameter schedule 80 high-strength steel pIpe with a 1 Q-inch and 12-inch helical
plate welded to the bottom lead.

Piers were driven into the ground using a hydraulic drive system to screw the piers into the soil. Pressure
readings of the hydraulic drive pressure, correlated to drive torque, were taken for each pier installed. Table 12
shows the maximum pressure and torque readings at the final installed depth for each pier.

After pier installation, Fasteel' Ultra pier brackets2 were Installed over the exposed pier shaft into recessed slots
cut into the footing, so that the pier bracket shelf solidly supported the stem wall The brackets were attached to
the foundation using two 5/8.inch expansive anchors.

Note that the bracket for pier 31 was a specially fabricated bracket designed to attached to the side of the stem
wall using *-inch steel plate and attached to the wall using 6 - %-inch diameter grade 75 all-thread bars affixed
to the wall using high-strength epoxy.
After installatIon of the brackets, hydraulic jacks were used to lift the foundation on the piers. A level was used
to determine floor level, and a torque wrench was used on the jacks to ensure even distribution of foundation
loading to the supporting piers.
After pier installation and foundation leveling, the areas surrounding the piers were backfilled and graded.
SUMMARY AND RECOMMENDATIONS
Based on our field observations and measurements, the piers, as installed, meet our design specifications for
capacity. We believe that the underpinning system, as installed, is adequate to support the design live and dead
loads for the foundation.
We recommend acceptance Of the foundation underpinning system, as-built, by the local building official.
