weldments and button punch on metal decking Welding was done With SMAW process uSing E7018 Electrode on all
structural steel components under decking The beams and tube steel columns were welded at fabncatlon shop
Without special inspection Weldments were painted at the time of inspection
,,\Ieldments were checked for Size, length, profile and location To the best of our knowledge, the work checked IS In
conformance With approved drawing, specification and applicable AWS D1 1 (Static) Code, except as noted for shop
weldments, which appear to be acceptable Contractor was asked to notify our office when additional services are
needed

Performed continuous inspection dunng the installation of 3/4" all thread epoxy anchored bolts In following locations
Lower base plates footing pad connections 8-(4) anchors each, at the garage approach Pnor to installation the
prednlled holes were cleaned by nylon brush and compressed air Venfled minimum embeddment depth of 4 Inches
The above listed epoxy anchors were Installed uSing a Epcon Ceramic 6 In accordance with the manufacturer's
directIons Contractor stated the nuts would be tightened after the epoxy has cured The epoxy anchors were Installed
In accordance with approved jobslte plans and specifications Contractor was asked to notify our office when additional
services are needed
