(CDF) in a trench on the above referenced lot '

cnp was supplied by Morse Bros and approKimately 20 C Y offill wa.g. placed In the trench
The top of the CDF was approXlnlRtely 10-12 inches below exlsting grade The ownerlbuIlder
informed us that extstmg grade was bottom of footing and th~ ronns In place were set consistent
with tills mformation
The line of the-footing will be approximately 3 feet (horizontal measure) from the excavatIOn that
was filled with COP ,.
In my professional opinion, based on my lDspectlon, the location of the forms on site and my
expenence in this area, the trench ,has been properly backfilled and no adverse effect to the footmg
~1l result from the excavatlon or backftlhng of this trench ^
