Performed continuous Inspection dunng the installatIOn of #5 epoxied rebar dowels In the follOWing locations Two (2)
pad footings to eXisting footings detail 4/S1 2, 6 #5's at gndllnes 1 at F 8 and G 8 Prior to installation the prednlled
holes were cleaned uSing a nylon brush and vacuum Venfled minimum embedment depth of 6 Inches The above
listed epoxy anchors were Installed uSing Simpson Set epoxy with ICC #ES-ER-1772 The epoxy anchors were Installed
In accordance with Job-Site plans and manufacturer's directions To the best of our knowledge, the work Inspected
above was In accordance with the bUilding department approved design drawings, speCifications and applicable
workmanship provISions of the International BUilding Codes Contractor was asked to notify our office when additional
servIces are needed
