from 4 to 4 5 and 5 to 5 5, 0 to 9'10" above footing Reinforcing steel and anchor bolts were checked for grade, Size, lap,
placement and clearance where applicable ReinforCing steel was placed In accordance with approved deSign draWings,
specifications, and revised detail SK-1 dated 9/18/07 All cells were relatively clean and mortar projections are within the
limits of the Uniform BUilding Code Performed continuous inspection dUring placement of grout In the locations listed
above Grout was placed by pump and mechanically consolidated All cells were grouted solid Note Clean-outs
prOVided To the best of our knowledge, the work Inspected above was In accordance with the building department
approved deSign draWings, speCifications and applicable workmanship provISions of the Uniform BUilding Codes
Contractor was advised of all on-site tests results and was asked to notify our office when additional services are
needed
