Performed continuous inspection during the installation of forty-six (46) #5 x 24" to 54"
vertical dowels into footing for extra reinforcement at corners for CMU wall. Prior to
installation, the predrilled holes were cleaned with a brush and compressed air.
Verified minimum embedment depth of 9". Dowels were secured using Simpson Set
22 (ICBO ES ER5279, expiration date 11-07). Note: holes were drilled using a 7/8"
diameter bit. Manufacturer's instructions state that a 3/4" diameter bit should be
used. Derrick (Meili Construction) was advised.

Changes to approved plans authorized by engineer: use of #5 epoxy anchored
vertical dowels verbally approved by engineer.
To the best of our knowledge, the work inspected was in accordance with engineer's
verbal instructions to Curt Dawson (Meiii Construction) and applicable workmanship
provisions of the I.B.C., except if noted above. Contractor was asked to notify our
office when additional services are needed:
