Performed final inspection on the reinforcing steel for 8" CMU wall on line A, from 3 to
6, on 6, from A to G (11'8" to 14'0"), Reinforcing steel was checked for grade, size, lap,
placement and clearance, Cells were relatively clean and free of excessive mortar
projections, Observed the placement of 4,5 cu.lyds, of MBI 7 1/2 sack grout (Mix 712-
G), Grout was placed by pump and was mechanically consolidated, Three (3)
masonry prisms were constructed for compressive strength testing. Grout slump g"+
grout temperature 68F; air temperature 56F, Walls were grouted solid,

To the best of our knowledge, the work inspected was in accordance with plans dated
06-29-06, latest revision dated 08-30-06 and applicable workmanship provisions of the
I.B,C" except if noted above, Cory Reiling (Haps Masonry) was advised of on-site test
results and was asked to notify our office when additional services are needed,
