1. Provide a preliminary title report issu1ed within the past 30 days documenting
ownership and listing all encumbranc,es. Also, provide a deed showing that Bill Dwyer
and/or Eureka Development, LLC is the property owner. If the applicant is not the
property owner, written permission fl-om the property owner is required.

2. Show the City boundary and Urban Growth Boundary that abut the property on the
plan set. I
3. Show the location, width, and type of all existing and proposed easements on and
abutting the property on the plan set.
4. Show the location of existing and prdposed street lighting, including type, height, and
area of illumination on the plan set.
5. Indicate the paving width and length for all existing and proposed driveways on the
plan set.
6. Indicate the size and type of the prolilosed street trees. Also, indicate which tree is
proposed to remain on the plan set.
7. Indicate the location, size, and type 0f stormwater management facilities on the site.
Also, indicate the location of the proposed underground power, tv, & phone lines and
connection points. If overhead pow~r is to be removed, show the lines as proposed
for removal on the plan set. In addition, indicate the sanitary sewer connection to
the existing house. Finally, provide ~ detail of the utility trench underneath the
proposed panhandle driveway.

PW Notes:
1. Provide an Oregon licensed Civil Engineer stamp with a signature on all required
sheets of the plan set.
2. Add a note to all sheets of the plan set indicating whether WQLWs exist on or
adjacent to the property.
3. Revise the TOTZ note on all sheets of the plan set to reflect the actual TOTZ.
4. Show the existing PUE along the High Banks frontage of the lot. Also, provide
clarification regarding the proposed easement.
5. The proposed sanitary sewer connections need revision. Each tax lot must be
served by a lateral from the public system. The 6" private line would need to be
changed to an 8" public line constructed under a PIP. Alternatively, the lots can
be served by parallel private laterals under the paved panhandle driveway.
6. Show elevations on the site assessment of existing conditions sheet.
7. Provide the required Stormwater Management System Study. As such, verify soils
are suitable for drywells and a rain garden/infiltration strip. Rooftop runoff for
new development should be routed to drywells, and vegetated stormwater
treatment for driveway runoff (rain garden, filter strip, etc.) must be proVided.
Finally, address discrepancies in the paved/easement clear widths.

This is not a decision on your application. Springfield Development Code Section 5.4-
105 and Oregon Revised Statutes 227.178 require the City take final action on a limited land
use decision within 120 days after the application is deemed complete. The 120-day
processing period for this application begins when all the missing information is submitted or
when you request that the City proceed without the information. You must indicate by either
signing this form or by submitting a written response to the City within seven days of the
date of this form asserting your intentions regarding the provision of the missing information.
If you indicate herein or in your written response that the missing information will be
submitted, then you have 180 days from the date the application was submitted for Pre-
Submittal Review to provide the City with the missing information. If you refuse to submit
the missing information, then upon receipt of the full application packet and processing fee,
the City will deem the application complete for purposes of starting the 120-day clock and
begin processing the application. No new information may be submitted after the start of
the 120-day period unless accompanied by a request for an extension of the 120-day
processing time. Upon receipt of a request for extension, the City may extend the 120-day
period for a reasonable period of time. The City may also require additional fees if the new
information is submitted after the Notification to Surrounding Property Owners is sent out
and a second notification is required or if the new information substantially affects the
application proposal and additional review is required.
