On November 27. 1996, your rmancial institute paid for ymlJ' construction permits for a single family
residence being constructed at 4536 Glacier street, Springfield. Oregon. .
On that date, you were notified, and you agreed to come to our office to sign your permits in order to
validate the permits. To date, you have not signed the required permits. please come to our office within
the next seven (7) days to sign the construction permits. Our office hours are Monday through Friday, 8:00
a.m. until noon and I :00 p.m. until 5:00 p.m.
