Subject: Fire Department referral at 225 Main Street, Springfield, Oregon
Dear Manager:
At the request of the Springfield Fire and Life Safety Department, the Building
Safety Division recently conducted an inspection at the above address. The
inspection revealed the following conditions which must be corrected in order for.
the building to comply with applicable City Codes. The following items must be
repaired, replaced, installed or modified:
Structural & Fire and Life Safety
1. Exposed paper and aluminum facings on insulation must be covered and in
direct contact with an approved surface material, such as 1/4 inch plywood
or 1/2 inch gypsum board; or the facing material must be removed from the
exposed insulation surface. The insulation facing materials are not approved
for exposed conditions due to their rapid flame spread characteristics.
2. There must be two properly maintained fire extinguishers with a minimum
rating..of 2-A, mounted in clearly visible locations with the top of the
extinguishers between 3 and 5 feet above the floor.
3. A clear aisle width of 44 inches must be maintained when merchandise is
placed on both sides of the aisle.
Electrical
4. Materials are obstructing the required 36 inch clearance in front of the
electrical service panel, which must be maintained for emergency access to
the panel.
5. Chain-hung light fixtures are improperly wired. Approved grounded
receptacle outlets are required directly above the fixtures, for direct
connection of the fixture cord.
6. Improper use of extension cords was noted which when used in place of
permanent wiring is hazardous and can cause electrical shock and potential
fire. NOTE: EXTENSION CORDS MUST NOT BE USED AS A SUBSTITUTE
FOR FIXED WIRING.

7. Unprotected open end wlrlng was observed, which reoquires approved
junction boxes and covers to prevent the potential for electrical shock and
the spread of fire under short circuit conditions.
8. One or more broken or missing cover plates were observed on outlets,
switches or junction boxes. All unused openings in electrical boxes must be
properly closed to prevent the possible spread of fire or shock.
9. Electrical conduits and devices and junction boxes were observed to be
loose, and must be properly secured to the structure.
Mechanical
10. Two ceiling hung space heaters have chimney connectors too close to
combustible material. A minimum of 6 inches clearance is required between
the chimney and all combustible materials.
11. The wood stove chimney connector has been reduced in size, This is not
permitted due to the increased temperatures which are produced at the
reduced connector.
12. The masonry chimney is not lined. A wood burning appliance may not be
connected to an unlined chimney where the masonry is less than 12 inches
in thickness.
13. The chimney does not have the capacity to accommodate both a wood stove
and a unit heater.
14. Unit heater #1 is not properly sealed at the masonry chimney inlet.
Due to the hazardous nature of items # 1 through 9, those corrections must be
completed within the next 15 days, or procedures necessary to cause the building
to be vacated may be initiated. The remaining items must be corrected within 30
days from the date of this letter.
Permits will be required for items # 1,5,7,10,11,12 and 14, or any additional
modifications you may desire which involve changes or additions to the struc-
tural, electrical, mechanical or plumbing systems. Required permits may be
obtained from this office.
Please note that installation and repair of electrical systems on property which
is intended for lease, sale or rent must be done by an electrical contractor who
is licensed by the State of Oregon.
