certify that any and all work performed shall be done in accordance with the Ordinances of the City of Springfield ond the lows of the State of Oregon
pertoining to the work described herin, and that NO OCCUPANCY will be made of any structure without the permission of the Building Division. 1 further
certify that my registration with the Builder's Board is in full force ond effect os required by ORS 701.055, that if exempt the basis for exemption is noted

Staff must be able to contact appropriate persons regarding
design information or job site corrections, etc.
II. Abbreviated Plumbing, Mechanical & Electrical 'Schedules
A. Except wnere blank spaces occur in the description portion
af the Mechanical and Electrical Schedules, the applicant
need fill-in anly the No. Baxes adjacent ta the appropriate
item(s) ta be installed
B. Full Plumbing, Mechanical, and Electrical Schedules
are available at the Building Division
1. To conserve space on the permit form the schedules
have been 'abbreviated
2. If the item(s) ta be installed are nat cave red an the abbrevi-
ated schedules you shauld consult the full schedules
C. BUILDING DIVISION STAFF WILL FILL OUT ALL FEES AND
CHARGES ON THE SCHEDULES

Whenever possiblerthe initial application will be used _/0-
warksh.e.et anly. Where possible, Building Divisian StaffIWill
prepare a typewritten copy and return it to the applicant at
the time the actual permit is issued for his signature.
IV. Fees and Charges
Plan check fees are due and payable at the time af the applicatian,
and na plans will be processed until these fees are paid. All
ather fees and charges are due and payable when the permit
is issued.
