CO.SY COpy ONLY BEFORE THE ELECTRICAL AND ELEVATOR BOARD OF THE STATE OF OREGON IN THE MATTER OF: JESS M BRANDT-DRURY, AN INDIVIDUAL ) ) ) ) ) ) ) ) ) ) NOTICE OF PROPOSED ASSESSMENT OF A CIVIL PENALTY RESPONDENT CASE NO. 2003-0664 ~ Pursuant to ORS 455.895(1)(b) and OAR 918-001-0036, the Electrical and Elevator Board proposes to impose a civil penalty against you in the amount of$2000 for the following violation(s): On or about the period of July 2003, at .!1lli-awnridge A venue, Springfield, Oregon, Respondent engaged in tht; business of an electrical contractor by installing electrical wiring, boxes, switches, outlets and fixtures, without a' valid Oregon Electrical Contractor's license, in violation ofORS 479,620(1), On or about the period of July 2003, at 1232 Lawnridge Avenue, Springfield, Oregon, Respondent installed electrical wiring, boxes, switches, outlets, and fixtures, in a residential structure, without a valid Oregon Electrical Supervising or Journeyman license, in violation of ORS 479.620(3), You are entitled to a hearing as provided by the Administrative Procedures Act, Oregon Revised Statutes, Chapter 183, If you wish to receive a hearing you must file a written request for a hearing with the Board. no later than 20 days from the date of service of this notice. If a request for hearing is not received within the 20 day period your right to a hearing shall be considered waived unless the failure to request a hearing was beyond your reasonable control.
