NOTICE OF PROPOSED
ASSESSMENT OF A CIVIL PENALTY
RESPONDENT
CASE NO. 2003-0664

Pursuant to ORS 455.895(1)(b) and OAR 918-001-0036, the Electrical and Elevator Board proposes to
impose a civil penalty against you in the amount of$2000 for the following violation(s):
On or about the period of July 2003, at .!1lli-awnridge A venue, Springfield, Oregon,
Respondent engaged in tht; business of an electrical contractor by installing electrical wiring,
boxes, switches, outlets and fixtures, without a' valid Oregon Electrical Contractor's license, in
violation ofORS 479,620(1),
On or about the period of July 2003, at 1232 Lawnridge Avenue, Springfield, Oregon,
Respondent installed electrical wiring, boxes, switches, outlets, and fixtures, in a residential
structure, without a valid Oregon Electrical Supervising or Journeyman license, in violation of
ORS 479.620(3),
You are entitled to a hearing as provided by the Administrative Procedures Act, Oregon Revised Statutes,
Chapter 183, If you wish to receive a hearing you must file a written request for a hearing with the Board.
no later than 20 days from the date of service of this notice. If a request for hearing is not received within
the 20 day period your right to a hearing shall be considered waived unless the failure to request a hearing
was beyond your reasonable control.
ORS 183.457 provides a corporation may be represented by an authorized representative at the hearing,
If you request a hearing the Board will notify you of the time and place of the hearing, The Board will
give you information on the procedures, right of representation and other rights of parties relating to the
conduct of the hearing before commencement of the hearing,
If you do not request a hearing within 20 days from the date of service of this notice or if you withdraw a
hearing request or fail to appear at a scheduled hearing, the Board may issue a Final Order by Default and
impose a civil penalty against you in the amount of $2000, Ifthe Board issues a Final Order by Default
against you, it will designate the Building Codes Division's file on the matter as the record,
