OAR Chapter 340, Division 7, Section 72.020(6): ThiS report shall remain In effect until Issuance of a
permit to construct, unless in the meantime conditions on thiS or adjacent properties have been altered In
any manner which would prohibit issuance of a permit, In which case thiS evaluation report shall be con.
sldered null and void. Technical rule changes will not invalidate any evaluation report Issued pursuant to
thiS section.
Before construction is started bring thiS form to the Department of EnVironmental Management and apply
for a building permit. Exact specifications for the sewage disposal system will appear on the permit
If the property IS only a portion of a tax lot, a partitioning or subdiVIsion must be approved before the
permit will be issued.
Only persons havmg a valid license Issued by the Department of Environmental Quality under ORS 454.695
to install subsurface sewage disposal systems or an owner or contract purchaser or hiS regular employees
may install subsurface sewage disposal systems. A certificate of satisfactory completion Will not be Issued
If persons other than the above perform the installation.

The factual statements set forth in the staff report are incor-
porated hereln and adopted. The Hearings OfficJal did not view
the site. In addition, the following flndings based upon the
testlmony presented at the hearing are adopted. The applicant
lndicated that the permit could be llmited to a moblle home.
Decision: CONDITIONAL USE PERMIT APPROVED. fureeconditions of
approval are imposed: -
1) The permit shall be for a period of 5 years.
2) Only a mobile home, a temporary str~cture or trailer
may be placed upon the property under this permlt. _
3) The permlt may be renewed automatically for an
addltional 5 years upon the applicant's written statement to the
Lane County Plannlng Division an accessory dwelllng for farm help
is stlll necessary and setting forth the specific reasons therefore.
Justlflcatlon for Decision (Concluslons):
The appllcation conforms to the comprehenslve plan for Lane County.
The proposed use will be compatible with and not adversely affect
the surroundlng vlcinlty nor wlll it ln turn be adversely
