Information hereon is true and correct, and I further certify
that any and all work performed shall be done in accordance
with the Ordinances of the City of Springfield, and the Laws
of the State of Oregon pertaining to the work described
herein, and that NO OCCUPANCY will be made of any
structure without permission of the Building Safety Division.
I further certify that only contractors and employees who
are In compliance with ORS 701,055 will be used on this
project.

INSTRUCTIONS TO BIDDERS
. All Contractors and subcontractors must have a current CCB license,
. The General Contractor shall be solely responsible for the design ,and functionality of all components of
Project. Approval by the City shall not imply any guarantee or certification of functionality,
. All work shall be performed to meet or exceed the City's Housing Programs Performance Reauirements and
Standard:,' unless contractor's desire to perform to a lower standard is clearly stated in the bid document.
City has the right, at its sole discretion, to reject any bid for work which will not meet program standards, A
copy of the program standards is attached to this document. Except as specified in tile bid document, City
shall not pay for work that does not meet its Performance Reauirements and Slandards,
. All required building permits and inspections are the responsibility of the General Contractor City shall not
, pay for work thathas not been permitted and approved by the City's Building Inspector (ifrequired),
. Bids must be complete, and include all items listed under REQUIRED WORK Items listed under
OPTIONAL WORK shall be bid separately, Prices shall be good for 30 days from the date of bid, A list of
materials must be attached to or incorporated into the bid specifications, Necessary plans, drawings and/or
designs shall be incorporated into the bid document at the time of submission,
. Bids must describe Contractor's policy regarding Change Orders, Change Orders must be approved by
Owner and City Housing Specialist.
. Funds will.be disbursed on the second Friday of each montll, Requests for Payment must be submitted for
approval no later than the 30th of each montll for disbursement on the second Friday of the following month,
City will pay only for work done, "payments in advance are not allowed, All requests for payment must be
approved by the owner and the City Housing Specialist.
. All work shall be completed no later than 90 days after start date,
REOUlRED WORK (neededfor structure to meet minimum housing quality stimdards)
I, Install facia board on rafter tails to accept gutters,
2, Finish rough opening on north gable-end of house, Use wood louvered panel or door to maintain adequate
attic ventilation,
3. Insulate attic area; use fiberglass baits ofR-21 or better. Lay batts on floor of attic (cciling of house),
Provide baffles as needed for proper ventilation, '
4, Install siding (T-lll, or similar) in place of roofing paper on short exterior wall.
5, Relocate door opening on north garage wall to be approximately in line with exis{ing house-to-garage door
Remove existing shelving as need to accomplish this activity, Install new melal insulated door, with new
securitylockset.
6, Frame in existing door opening. Remove "stub wall" on northeast comer of garage. Install siding (T-III or
similar) in place of plywood and any defective siding on north side of garage,
7, Remove existing window on east garage wall. Reframe opening and install new vinyl insula{ed slider of
suitable size to provideadequate-egress, .
8, Remove "dead" electrical conduit on garage wall prior to painting,

9. Prepare exterior for painting, Remove all loose and defective paint, sand and fcather cdges where neccssary
to ensure proper adhesion, Apply one coat primer. two finish coats to body, Trim (windows, doors, gable-
end bracing, etc,) in contrasting color. All colors to be of owners choice,
10. Install continuous metal gutters, owner's choice of standard colors, Use splash blocks to divert water away
from building, walkways,
II, Install new gas furnace of suitable capacity in living room, Contractor is responsiblc for obtaining all
applicable permits, Use existing chimney, replace non-confonning flue system,
12, Install electric wall heater of suitable capacity in bcdroom,
[3, Install GFI circuit in bathroom, Permit required.
14, Relocate clothes dryer outlet next to washing machine, Vent dryer under landing and through west garage
WotIL Electrical and mechanical permits required,
15, Redesign and build landing, steps and railings in garage, Configure to provide straight access to new door.
16, Remove dead tree in front yard, Extract stump, or alternatively. grind stump to ground level.
17, Repair broken window pane in bathroom,
18, Repair garage floor. Concrete patch must be doweled (rebar) 10 existing floor to prcvent independent
movement of patch. Contractor is not responsible for repair of floor. covering,
