Notice - Failed Final Site Plan Inspection
Project Name: VoiceStream/Osburn Wireless WTS Facility
Project'Proposal: Site Plan for 100' WTS monopole with 9 antennae and ground-based
equipment.
Case Number: DRC2005-00067
Project Location: The subject property is located at 3950 Kathryn Street, Assessor's Map
17 -02-30-44-00; Tax Lot 900
Zoning: Heavy Industrial (HI)
Overlay Districts: Drinking Water Protection Overlay District.
Applicable Refinement' Plan and Designation: Mid-Springfield - HI
Metro Plan Designation: HI
Application Submitted Date: September 27, 2005
Decision Issued Date: November 15, 2005
Planning staff performed a Site Inspection on August I B, 2006 and found the following items to
be out of conformance with the approved Final Site Plan:
I, A generator and fuel tank have been installed, Condition 7 of the land use decision
specifically required these items to be deleted from the proposal OR the applicant was required
to seek approval for the use of fuel within the City's Drinking Water Protection Overlay
District, No MSDS for the generator fuel were submitted ana the applicant chose to eliminate
the generator and tank from the Final Site Plan, The generator and the tank have not been
approved,
2. The required landscaping is dying, Approximately six arborvitae around the fenced enclosure
and four cedars along Olympic Street are brown and appear to be dead/dying,
Until these items are resolved, the operation of the wireless communication facility is in
violation of the Springfield Development Code, Please contact Linda Pauly at the City of
Springfield (S41) 726-460B to discuss how these outstanding issues will be addressed in order to
bring the project into compliance,
The landscaping must be replaced and the generator and tank must be removed; OR the
applicant must submit a Site Plan Modification application to seek approval for the generator and
tanks, According to the City's Wellhead Protection Maps, the subject site is located within the
City's Drinking Water Protection (DWP) Overlay District, I yr-5yr TOrr Maia Wellhead,
Therefore, use and storage of generator fuel must be in conformance with Springfield
Development Code Article 17 and may also require approval of a Drinking Water Protection.
