Performed visual Inspection of reinforcing steel In the following locations Stryrofoam block walls at living area top ot'
footing to 6' garage walls top of footing to 5' and Intenor wall between garage and living area top of footing to 4'
Reinforcing steel was checked for grade, Size, lap, placement and clearances where applicable R,elnforclng steel was
secured uSing plastic posltroners. As directed by owner/contractor, contInuous inspectIon dunng placement of concrete
was not required Cast one set of three (6x12) concrete test cylinders Note As per conversation between owner Mike
Ferchland and Tim with Great House Design (2) 5/8" anchor bolts could be placed at 32" on center max at floor ledger
To the best of our knowledge, the work Inspected above was In accordance wIth the bUilding department approved
design drawings, speCifications and applicable workmanship provISions of the Uniform BUilding Code Contractor was
'd to notlfy,our office when additional services are needed
