SCOPE OF WORK
Contractor will:

1. Remove sheetrock on N. W. wall and ceiling of garage as needed and temporarily support
roof framing. ~
2. Remove and replace sheetrock and framing of pony wall at back of garage approx. 13 'x6",
further inspect floor framing for additional damage.
3. Remove approx. 22'xlO' section of interior garage slab.
4. Remove approx. 22'x6' section of concrete along outside of garage wall.
5. Remove 8 panels on driveway approx. ITx30'.
6. Demo and remove damaged N.W. concrete gradebeam approx. 19'x2'x8".
7. Install 3 (three) FASTEEL@ 27/8 Titan@ Helical Piers with pre-construction t-caps,
per engineer's repair plan.
- Bid based on maximum depth of 12 (twelve) feet per pier.
- Additional footage to be billed at $20 per foot per pier.
8. Straighten brick column to plumb at corner of garage.
9. Re-frame exteriorN.W. wall with 2"x6" lumber, tape and texture sheetrock, siding and trim
as needed.
10. Form and pour new gradebeam per engineer's plan.
11. Prep soil and replace garage slab, outside parking area and driveway sections.
)) Permit for mentioned work included in Total Contract Price.
)) Section of back patio to be matched as close as possible with new concrete.
)) Licensed contractor to remove and replace furnace and water-heater as needed.
)) Contractor will attempt to straighten the garage corner brick fa~e. If cvul...ctor
cannot achieve plumb, contractor to reevaluate fIX with owner and implement
fix via written change order.
)) Painting not included in scope of work.
)) All F ASTEEL@piers covered under a 10 (ten) year warranty.
TOTAL PRICE: $23,300
)) If this proposal is not accepted within 30 days, price is subject to change.
The client understands and accepts that the contractor is not responsible for interior
plaster/drywall, exterior siding or landscaping damage caused by the normal course of installation
of F ASTEEL@ piers and cannot be held liable for same.

All material is guaranteed to be as specified, and the above work to be performed in accordance
with the drawings and specifications submitted for the work and completed in a substantial
workmanlike manner for the sum of exactly Twenty-Three Thousand Three Hundred Dollars, to
be tendered as scheduled below:

Balance due upon project completion
Van Dorn Construction, Inc. certifies upon Client's final payment that material suppliers, labor &
sub-contractors used on this project have been paid in full.
Any alteration or deviation from the above specifications involving extra costs, will be executed
only upon Client's authorization, and will become an ,,"'J.... charge over the contract price, billable
on a Time & Materials basis at a labor rate of$35 per hour and material rate at cost plus (+) 10%.
Any deviation from the above mention "scope of work" involving additional piers, shall be
defmed in a written "change of contract" to be signed and agreed upon by both parties before
additional work is completed. This agreement '''p,,,sents the entire understanding of Client and
Contractor as to those matters contained herein. No prior oral or written understanding shall be of
force or effect, with respect to those matters covered hereunder. This agreement may not be
altered except in writing signed by both parties.
All disputes hereunder shall be resolved by binding arbitration in accordance ~ith rules of the
American Arbitration Association. \
