Client: Mr Terry Fitzpatrick Project: IMRT FacIlity Date: 05-08-07 Oregon Urology Institute Urology Healthcare Project No.: 2077063 2400 Hartman Lane 1457 G Street Report No.: E-16240 Springfield, Oregon 97477 Springfield, Oregon Inspector: M Meyer ID No.: Inspection of: Sltework Weather: Clear/BOo Continuous Inspection: 0 Contractor: McKenZie Commercial B.P.No. COM2007 -00367 Periodic Inspection: 0" Met on-site With Gary Thompson (McKenZie Commercial) to observe In-progress excavation operations in the prevIous UST removal area between lines F and Hand lines 2 and 3 5 Currently, excavation IS approximately 90% completed, working from east to west uSing a John Deere 490E track-hoe equipped With a :t36" Wide smooth bladed bucket Excavation IS being terminated at an apparent ballast slab approximately 11 5 feet below eXisting pavement grade Note native gravel strata described In the project SOils report is VISible In the Sidewall at the northeast corner of the excavation an slightly above the top of slab at approximately 11 0 feet below eXisting pavement grade Emerald Excavating crew has stockpiled usable sandy bar-run gravel removed from the UST excavation In the proposed north parking lot to raise grades In prevIous bUilding area UnSUitable and/or saturated excavation matenals are being hauled off-Site Based on our observations of area around the stockpile, It appears that the eXisting loose, unSUitable surface matenals were removed to an elevation of :t458 00' Geotextlle fabriC was placed on the broWnish black Silty clay and excavated material was then stockpiled on the fabrlced area Discussed proposed fill operations With Mr Thompson and Doug (Emerald) A de-watenng pump IS available and Imported 1 1/2"-0 crushed rock IS being stockpiled for use as backfill once excavation IS completed Obtained a sample for mOisture/density determination and scheduled next site VISit for mid-morning, tomorrow, to perform field compaction testing .
