Performed inspection on the reinforcing steel In 8" CMU north wash tunnel wall, north,
east and west wall of office/break room area and Intenor tunnel arches (8'8" to top),
east and west tunnel arches (8'0" to 12'8") Reinforcing steel was checked for grade,
Size, lap, placement and clearance, where applicable Cells were relatively clean and
free of excessive mortar projections Observed the placement of 10 0 cu /yds of MBI 7
sack grout (MIX 7-G) In walls noted above Grout was placed by pump and was
mechanically consolidated Walls were grouted solid On-site test results are as
follows slump 8+", grout temperature 70F, air temperature 62F
Changes to approved plans authorized by architect: reinforcing steel clanflcatlon
memo dated 06/29/06
To the best of our knowledge, the work Inspected was In accordance with plans dated
04/14/06 and applicable workmanship provIsions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
