Reinforcing steel was placed In accordance With approved deSign draWings, specifications, and latest revIsions dated
July 10, 2006 at wheel ring Performed visual Inspection dUring placement of concrete m the above locations Concrete
was placed by truck chute and mechanically consolidated Cast one set of four (6x12) concrete test cylinders Note
verified rebar clearances prior to concrete pour on wheel ring Will perform final Inspection on rebar at north truck
foundation prior to pour. To the best of our knowledge, the work Inspected above was In accordance With the bUilding
department approved deSign draWings, speCifications and applicable workmanship prOVISions of the International
BUlldmg Code Contractor was asked to notify our office when additional services are needed
