Performed continuous Inspection dUring the InstallatIon of four (4) 1/2" x 6" stainless
steel all-thread anchors Into slab for generator anchorage Prior to Installation the pre-
drilled holes were cleaned With compressed air and a brush Verified embedment
depth of 4 Inches Anchors were secured With Simpson AT epoxy (ICBO ER 5791,
expiration 03/07) Contractor stated that epoxy would be allowed to cure before nuts
would be tightened
To the best of our knowledge, the work Inspected was In accordance With plans dated
05-09-06 and applicable workmanship prOVIsions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
