Periodic Inspection:
Performed final Inspection on the reinforcing steel for the perimeter and Interior
continuous footings and interior F2 column footings A preliminary Inspection was
performed on 07-07-06 (ref our report no E13219) Observed the placement of 465
yards of ESG 4000 pSI (*4000 psi required) concrete (mix 2146) Into area noted
above Concrete was placed by pump and was mechanically consolidated Cast one
(1) set of four (4) 6" x 12" concrete test cylinders Notified Gary (McKenZie
Commercial) of the follOWing on-site test results slump - 5 1/2", concrete temperature
- 7rF, and air temperature - 730F
To the best of our knowledge, the work Inspected was In accordance With plans dated
04-07-06 and applicable workmanship provISions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
