Performed final Inspection on reinforCing steel With anchor bolts In penmeter stem walls
for bUilding Within lines D to K, from 2 to 12 Preliminary reinforCing steel inspection
was performed by our office on 07-21-06 (ref report no E13329) Observed the
placement of 40 yards of ESG 4000 pSI concrete (mix 2146) In stem walls noted above
and rat slab on grade on Intenor of bUilding Concrete was placed by pump and
mechanically or manually consolidated Cast one (1) set of four (4) 6" x 12" concrete
test cylinders from load no 1 Notified Gary (McKenZie Commercial) of the follOWing
on-site test results slump 4 1/4", air temperature 580F, and concrete temperature
800F
To the best of our knowledge, the work Inspected was In accordance With plans dated
04-07-06 and applicable workmanship provIsions of the I B C , except If noted above
Contractor was asked to notify our office when additional services are needed
