Performed final Inspection on the reinforCing steel for slab on grade With thickened
areas and slab on deck between lines D and C and between 2 and 12, and four (4)
extenor pedestals and footings Preliminary reinforCing steel inspection was performed
by our representative on 08/10/06 (ref FEI report no E13832) Observed the
placement of :t100 0 cu /yds of Eugene Sand and Gravel's 4000 pSI concrete (MIx
2146) In areas listed above Concrete was placed by pump and was mechanically
consolidated Cast one (1) set of four (4) 6" x 12" concrete test cylinders from load 4
Gary (McKenZie Commercial) was notified of on-site test results slump 6", concrete
temperature 74F, air temperature 50F
Changes to approved plans authOrized by engineer: contractor stated EOR has
changed concrete requirement to 4000 pSI for slabs
To the best of our knowledge, the work Inspected was In accordance With plans and
applicable workmanship proVISions of the I B C , except If noted above Contractor was
asked to notify our office when additional services are needed
