Inspection of. Reinforced Concrete Weather: Clear Continuous Inspection. 0
Contractor: Melli Construction BP No.: COM2006-00624 Periodic Inspection' 0
Observed placement of sixtY-nine (69) yards of MBI fiber reinforced concrete (mix
24F4NE5200) for slab on grade on lines A-D and 1-6 Fiber reinforced concrete was
used In lieu of reinforcing steel as per detail 11/S5 3 Concrete was placed by pump
and manually consolidated Concrete was sampled from load 3 and one (1) set of four
(4) 6" x 12" concrete test cylinders were cast Concrete temperature 630, air
temperature 400, and slump 5 1/2" Derrick (Melli) was notified of on-site test results
and was asked to notify our office when additional services are needed
To the best of our knowledge, the work Inspected was In accordance with reVised plans
dated 08-30-06 and applicable workmanship provIsions of the I B C , except If noted'
above Contractor was asked to notify our office when additional services are needed
