Inspection of: Reinforced Concrete Weather' Overcast Continuous Inspection' 0
Contractor: Melli Construction BP No.: COM2006-00624 Periodic Inspection: 0
Performed final Inspection on the reinforcing steel for penmeter stemwalls A
preliminary inspection was performed on 10-30-06 (ref FEI report no E14545)
Reinforcing steel was checked for grade, Size, lap, placement and clearance
Reinforcing steel was secured with tie-Wire Observed the placement of 10 5 cu /yds
of MBI 3000 pSI concrete (MIx 30-5PV) In perimeter stemwalls Concrete was placed
by pump and was mechanically consolidated Cast one (1) set of four (4) 6" x 12"
concrete test cylinders Curt Dawson (Melli Construction) was notified of on-site test
results slump 5", concrete temperature 63F, air temperature 40F
To the best of our knowledge, the work Inspected was In accordance with plans dated
06-29-06 and applicable workmanship provIsions of the I B C , except If noted above
Mr Dawson was asked to notify our office when additional services are needed
