footmgs - along line 6, from lines D 9 to G, on D 9, from 6 to 5 9, on 5 9, from D 4 to
D 9, on D 4, from 5 4 to 5 9, on 5 4, from D 1 to D 4, on D 1, from 5 4 to 6, on 6, from
D 1 to A, and on A, from 35 to 6 Column footmgs - at lines A4, A5, A6, C6, D4, D5,
D6, E4, E5, E6, F2, F3 and F5 Stairwell footmg around elevator pit - on lines 2 3, from
Imes E 2 to E 5, on E 5, from 23 to 3 7, on 37, from D 6 to E 5, and step footing along
line E 2, from stairwell footing to elevator pit Relnforcmg steel was checked for grade,
Size, lap, placement and clearance Rebar was secured with tie-wire and supported
above grade Observed the placement of eighty-one (81) yards of MBI 3500 pSI
concrete (mix 35-1 KV) at the above noted locations Concrete was placed by pump
and consolidated by mechanical vibration Cast one (1) set of four (4) 6" x 12" concrete
test cylinders Curt (Melli Construction) was notified of the fOllowmg on-site test results
concrete temperature 620F, air temperature 530F, and slump 3 1/2"
Corrections of previously-listed items and uncorrected previously-listed items
as per report E 14405, dated 10-13-06, at area of step m footing line 3 6 from D 5 to
E 5, bent bars were m place Second set of diagonal bars not Installed Curt from Melli
Construction stated he had verbal okay from EOR to omit bars Wntten authorization
pendmg
To the best of our knowledge, the work Inspected was m accordance with plans dated
05-30-06 and Farwest Steel plans dated 09-26-06 and applicable workmanship
provIsions of the I B C , except If noted above Curt (Melli) was asked to notify our
office when additional services are needed
